REGISTRATION INFORMATION

All teams that are competing in the Elite Invitational must present the proper, fully-completed paperwork in order participate in the event. TEAM MANAGERS MUST REGISTER THEIR TEAMS A MINIMUM OF ONE HOUR PRIOR TO THEIR FIRST CONTEST ON SATURDAY MORNING - AT THE HEADQUARTERS TENT AT THE SITE OF THEIR FIRST MATCH.
Accepted teams have access to the Team Registration Checklist – which is designed to make sure that you bring all the paperwork necessary to register your team.
US Youth Soccer Requirements:
If carded under a US Youth Soccer State Association, you will need to provide the following:
1. Your Original, State-Approved Roster - and one extra copy of that roster for the tournament,
2. Validated Player Cards - for both rostered players and guest players,
3. Validated Coaches Card - one for each coach - maximum of 3,
4. Valid Medical Release Forms - one for each player. You may use your team’s own/existing forms,
5. The completed Tournament Liability Release Form - signed by a parent or guardian for each player,
6. Approved Travel Permit - if from outside of Ohio-South…
7. Approved Guest Player Roster - approved and validated by the state in which the player is registered
8. Your completed Hotel Information form
US Club Soccer Requirements:
If carded under US Club Soccer affiliation, you will need to provide the following:
1. Your official, Approved Roster,
2. A validated Player Card for each player,
3. A validated Coaches Card - one for each coach - maximum of 3,
4. A valid Medical Release Form for each player. You may use your team’s own/existing forms,
5. The completed Tournament Liability Release Form - signed by all players and a parent or guardian
6. Your completed Hotel Information form.
Midwest Regional League Team Requirements:
This is a four-match tournament, and the maximum number of MRL matches allowed in a weekend is three (3). Thus, each team will play at least one non-MRL contest during the tournament, and must provide the Permission to Travel form (if a US Youth Soccer carded team). If your fourth match happens to be against another MRL team, it will not be an official MRL match, - instead, it will be a tournament match. In addition to the items from the appropriate heading above, the MRL teams also must bring:
1. Your completed MRL Spring 2008 “Check-In Information” form, (NOTE: Ignore "Referee Fees" section - which were paid by your tournament entry fee)
2. A completed MRL Match "Line-Up Card" for each MRL match your team will play during the tournament,
3. A completed MRL Match "Referee Report" card for each MRL match your team will play during the tournament, and
4. Your completed and approved MRL Guest Player Roster, if applicable.
